Facility in Mobile, AL seeking Geriatric NP locums coverage
Rates: $110/hr
Program is seeking a Locum Nurse Practitioner to provide primary medical coverage in collaboration with PO physicians. The role is essential in delivering a full spectrum of health care services to frail elderly individuals who require nursing home care but are served in their communities whenever possible.
Responsibilities:
Provide primary medical care to PACE participants, including initial, semi-annual, and annual assessments.
Manage acute and chronic conditions, prescribe medications, therapies, and treatments as needed.
Participate in discharge planning and coordinate with the Medical Director and Clinic Supervisor on clinical policies and standards of care.
Make nursing home and/or home visits as necessary.
Advise on ways to improve service delivery and accountability.
Participate in provider on-call rotation as required.
Qualifications:
Master's degree in Nursing (REQUIRED)
Minimum of Five (5) years of experience, preferably in long-term care (REQUIRED)
Active Alabama License (REQUIRED)
BLS, CDS, DEA Certifications (REQUIRED)
Board Certified by ANCC or AANP (REQUIRED)
Certification as a Nurse Practitioner with prescriptive authority (REQUIRED)
Experience working with the frail or elderly population (Minimum of 1 year strongly preferred)
Valid driver’s license, proof of insurance, and means of transportation (REQUIRED)
