• Any State License accepted but provider must be willing to obtain OK license if accepted
• Open Until Filled
• Period of Performance: ASAP for six (6) months
• 14 shifts per month
• 7a-7p; 8a-8p; 12a-12p or 7p-7a
• Travel, lodging and malpractice paid
