Job Description: Nurse Practitioner, Primary Care
The Nurse Practitioner, specializing in primary care, will work closely with the Medical
Director (MD) to provide excellent clinical services. They will direct, implement, and provide a plan of care for patients at clinics.
They will provide general preventative care, conduct check-ups, treat both acute and
subacute illnesses, order lab tests, and prescribe medication for patients.
Clinical direction and oversight from Physician.
Key Responsibilities:
Performs physical exams and patient observations
Records patients' medical histories and symptoms
Creating patient care plans and contributing to existing ones
Ordering, administering, and analyzing diagnostic tests
Monitoring and operating medical equipment
Diagnosing health issues and plan of care for patients
Administering medicine and other treatments
Detecting changes in patients' health and modifying treatment plans, as needed
Consulting with healthcare professionals and families
Training patients and their families to manage and prevent illness and injury
Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities
Facilitates the patient’s transition within and between health care settings, e.g. admitting, transferring, and discharging patients
Facilitates staff, patient and family decision making by providing educational tools
Monitor and provider start-up status of clinics
Provide coverage to additional clinic locations, as needed and assigned
All other duties as assigned
Education & Experience:
A minimum of 2 plus years’ experience in family medicine as NP/PA
Graduate of an accredited school of nursing
Graduate of an accredited Nurse Practitioner Program
Current Registered Nurse Licensure in State of Texas
Current APRN/Nurse Practitioner Licensure in State of Texas
Current Board Certification in adult primary care or family practice specialty area
Current Medicare provider enrollment with active PTAN number
