Job description

Practice Setting: Clinic
EMR: E-Clinical
General Internal Med cases; Chronic conditions, Acute care, underserved patients.
Must be comfortable working with LGBT patients
Provide health maintenance, chronic disease co-condition management and HIV specialty care for patients living with HIV infection/AIDS.
Coordinate services for patients living with HIV/AIDS with the support of registered nurses, medical assistants, health educators, and case managers.
Provide HIV pre-exposure prophylaxis (PrEP) to persons at risk for HIV infection and HIV post exposure prophylaxis (PEP) for persons with recent HIV exposure.
Serve as the HIV clinical consult for all staff within the agency.
