Job Description

The primary focus of the Nurse Practitioner or Physician Assistant will be to aim to improve the health of the community while enhancing interactions with patients, providers, and employees.

Details:

Permanent, full-time position
Monday through Friday schedule
Work in a new transitional care clinic
Assist in directing patients to primary care, specialist providers, community pharmacists, and other care providers as appropriate
100% outpatient
Approximately 10 patients per day
