We are looking for a Physician Assistant or Nurse Practitioner to join our group full time as our Clinical Program Coordinator in South Bend, IN. The Clinical Program Coordinator is responsible for assisting Clinical- Operations with the ongoing assessment and improvement of the Clinical Services Team. Such responsibilities encompass not only the internal functioning of the department, but also the integration of the department into the overall functioning of the organization and its clinical programs.
The Clinical Program Coordinator (CPC) is the primary resource leader for designation location(s) and shall oversee and work in collaboration with the customer and clinical team members to ensure; optimal functioning and performance of the clinical program(s) and their components and day to day clinical operations as directed by their supervisor. The CPC will also assume responsibility and accountability for ensuring patient evaluation and management is completed in accordance with established policies and procedures and maintaining the highest degree of quality patient care. The CPC assumes and is responsible and accountable for the care of patients requiring assessment and treatment within the program. When applicable may be assigned to orient staff and students. The CPC accurately streamlines patients’ needs; assists in identifying and scheduling appropriate clinical interventions; provides patient and family education; and works cooperatively with other clinical team personnel in maintaining standards for professional clinical practice and interdisciplinary teamwork.
