Contract Length: 3-6 months with option to extend, start upon credentialing
Shift Details: M-F 9a-5p
Specific Functions:
Primary care/health maintenance health screenings in the community, patient education about screening results and need for follow up, referrals to the adult medicine/peds clinic to establish care with a primary care provider and "gaps in care" services. Offered on a rotating basis in our service area.
HIV: Serve as the primary provider for all newly diagnosed, new to care and returning to care patients living with HIV. Serve as the clinical consult for staff with HIV related questions. Provide routine (quarterly) staff updates and in-service to all clinical staff. Serve as the provider for all DHEC PrEP referrals. Meet with all HIV positive patients annually for a HIV focused annual wellness visit
Ryan White: Serve as the RW CQM committee lead. Hold monthly care conferences with MCMs and providers on select clients to ensure all client needs are met, identify barriers to care and increase retention and viral load suppression.
Administration/Outreach: Meetings, leadership duties, charting f/u, health fairs
