We are Seeking a dedicated Permanent Adult or Family Nurse Practitioner to join our long-term care team, providing comprehensive and compassionate healthcare services to adult patients. RESPONSIBILITIES
Provide comprehensive healthcare services to adult and elderly patients in long-term care settings.
Conduct thorough assessments and develop individualized care plans for patients with chronic and acute conditions.
Collaborate with interdisciplinary teams to ensure continuity of care and optimal patient outcomes.
Prescribe medications and treatments in accordance with state laws and institutional protocols.
Educate patients and families on disease management, preventive care, and healthy lifestyle choices.
Monitor and evaluate patient progress, adjusting care plans as necessary to meet evolving needs.
Maintain accurate and up-to-date patient records, ensuring compliance with healthcare regulations and standards.
QUALIFICATIONS
Possession of a valid and unrestricted Nurse Practitioner license in the state of practice.
Completion of a Master's or Doctoral degree in Nursing from an accredited institution.
Certification as an Adult or Family Nurse Practitioner by a recognized certifying body.
Extensive experience in long-term care settings, demonstrating clinical proficiency.
