Assignment Dates:

Location: INDIANA - Cambridge City, and Liberty

MONDAYS = LIBERTY – HOURS 6:30a-12:30p

TUESDAYYS = CAMBRIDGE – HOURS 8:00AM-7:00PM W/ 1 HR LUNCH

WEDNESDAYS = SPLIT DAY – CAMBRIDGE 7:30AM-11:30AM then to LIBERTY 12:30PM-6:30PM

THURSDAYS = LOCATION AND TIME TBD

JOB REQUIREMENTS:

Completed Master’s Degree Level Nurse Practitioner Program or Physician Assistant Program from an accredited college/university
Active unrestricted applicable licensure
CPR Certification
NPI & DEA
Current National Board Certification
EXPERIENCE:

Prefer OCC Health Experience
Ideal candidate will have at least three (3) years primary care practice experience, the ability to work independently, and experience working with and Electronic Medical Record.
