Job Title: Nurse Practitioner & Physician Assistant - Primary Care/Family Medicine/Internal Medicine
EMR: Advanced MD
Patient Volume: 15–20 patients per day
Hours: Monday to Friday, 9 AM – 6 PM (may include occasional evenings or weekends)
Physical Demands: Standing, walking, and patient lifting
Job Summary:
The Nurse Practitioner (NP) in this role provides comprehensive care to patients of all ages in a primary care or family medicine clinic. Responsibilities include diagnosing, treating, and managing acute and chronic health conditions, collaborating with the healthcare team, and ensuring quality patient care, timely documentation, and accurate billing for services.
Key Responsibilities:
Assessment and Diagnosis:
Perform physical exams and obtain patient histories.
Order and interpret diagnostic tests.
Develop and manage treatment plans.
Treatment and Management:
Prescribe medications and therapies.
Educate patients and families on health management and prevention.
Care Coordination:
Collaborate with healthcare providers and facilitate specialist referrals.
Implement care plans and follow-up strategies.
Documentation:
Maintain accurate patient records in EHR systems.
Ensure compliance with documentation standards.
Quality Improvement:
Participate in quality improvement initiatives.
Stay updated with best practices and guidelines.
Qualifications:
Education: Master’s or Doctoral degree in Nursing (MSN, PhD, DNP).
Certification: Board certification as FNP or AGPCNP; credentialing at local hospitals.
Licensure: Valid RN and NP license in the state.
Experience: 3 to 5 years of experience, preferably in a primary care or family medicine setting.
Skills:
Strong clinical, communication, and interpersonal skills, with proficiency in EHR systems.
Working Conditions:
A clinic setting with occasional hospital inpatient rounds.
